I have read the Parent Information Sheet and have had an opportunity to ask the researchers any further questions I may have. On the basis of that information, I understand that there are no expected risks to myself or my chil  as a result of us participating in this study. I understand that our participation in this research is completely voluntary and I, or they, may decline to participate without affecting our relationship with the researchers, Early Start Discovery Space or the University of Wollongong.

I understand that if assigned to the ‘program’ group, mine and my child’s participation in this research will involve:

  1. Collection of parent and child assessment data in April 2021 taking about 90 minutes;

  2. Attendance of online sessions via zoom (or viewing of recording prior to workshops) lasting 30 minutes.

  3. Attendance of in-person workshops, 4 times, acros  May - June 2021, held every other week for approximately 90 minutes.

  4. At-home play activities twice a week in-between in-person workshops

  5. Collection of parent and child assessment data again in June/July 2021 to evaluate any changes that may have occurred.

Total time required from April to July 2021 = 15 hours

I understand that if assigned to the ‘control’ group, mine and my child’s participation in this research will involve:

I understand that I may also be contacted in the future to participate in further phases of the study, that I may agree to or decline at the time. I understand that participation in this phase of the research does not obligate me to participate in any future research that has not been outlined in the Parent Information Sheet.

I understand that the data collected will be reported in anonymous, aggregate form in academic journals and conferences. Nevertheless, at all times confidentiality is assured. I understand that all information collected will be anonymous, will be kept strictly confidential and that my child will not be identified in any part of the research.

I understand that I will recieve a $20 Discovery Space voucher only on completion of the post-program data collection at the end of the project.

By signing below, I am indicating my consent for myself and my child to participate in this research.
